What contrast actually does to the scan
Gadolinium is a paramagnetic metal that, when bound into a chelate complex (the form approved for clinical use), shortens T1 relaxation time of nearby protons. On T1-weighted MRI sequences this produces brighter signal in vascular structures and in tissues with leaky blood-tissue barriers, such as tumours, areas of active inflammation, infection, and post-surgical scar tissue. The contrast is administered intravenously, usually through a cannula placed before the patient enters the scanner; sequences are acquired before and after administration to allow direct comparison.
For specific indications, contrast is essentially diagnostic. Brain MRI for suspected tumour is non-diagnostic without contrast (because lesion enhancement is the most reliable feature distinguishing aggressive lesions from benign). Multiple sclerosis MRI uses contrast to identify acutely enhancing plaques, which carry different prognostic and treatment implications than chronic non-enhancing plaques. Breast MRI uses dynamic contrast-enhanced timepoints to characterise lesions by kinetic enhancement pattern, which is the basis of BI-RADS MRI lesion categorisation.
For other indications, the without-contrast study is the clinical default and contrast adds little. Most extremity MRI (knee, ankle, wrist, shoulder for routine pathology) is performed without contrast and the radiologist read is fully diagnostic.

- Brain MRI for tumour, infection, MS, post-surgical follow-up.
- Breast MRI for any diagnostic indication. Bilateral with-contrast is the standard.
- Pelvic MRI for cancer staging, endometriosis mapping, fistula assessment.
- Abdominal MRI for liver-lesion characterisation, pancreatic mass, biliary obstruction workup.
- MR angiography and MR venography of any vascular territory.
- Spine MRI for post-operative scar versus recurrent disc, suspected discitis or epidural abscess.
- MR arthrogram of any joint (uses dilute intra-articular gadolinium, separately to IV gadolinium).
For each of these indications, refusing contrast does not save money on net: the without-contrast study would not answer the clinical question and the scan would need repeating. The clinical-question framing matters more than the line-item cost.
