US Imaging Centers: 12,260 MRI and Radiology Centres by State and Metro

Every fixed-site MRI and radiology centre in the CMS National Provider Identifier registry, grouped by state and metro area, with the registry phone number, the date the record was last updated, nearest alternatives and the cash-price context for that market. 1,747 of the 12,260 records carry the MRI-specific taxonomy code.

Key insight: the cheapest scan is almost always at a freestanding centre, and this is the only national list of them that is public, free and complete. It is an identifier registry, not a licence register, so use it to find who to call, then verify.

Imaging Centres by State

Records per state in the August 2026 file. "MRI code" counts organisations that registered the MRI clinic/center taxonomy; "metros" counts the Census metro and micropolitan areas with at least two records (each has its own page). Centres outside those areas are listed on the state page.

How to Use the NPI Registry to Find Cash-Pay Centres

The National Provider Identifier registry is the one public list that every imaging business in the US is on, because you cannot bill Medicare or most insurers without an NPI. Here is how to read it for cash-pay shopping.

  1. Start from the taxonomy code, not the name

Organisations registered as 261QM1200X (MRI clinic/center) told CMS they operate MRI; 261QR0200X (radiology clinic/center) covers broader outpatient imaging. Chains such as SimonMed and Envision often register under the physician code 2085R0202X instead, which is why this directory also carries name-classified records, flagged as such.

  2. Prefer freestanding organisations over hospital subparts

NPPES marks records that are subparts of a larger organisation. Hospital-owned outpatient imaging usually bills hospital rates even in a strip-mall building; we exclude hospital subparts unless the record itself is the imaging entity, and we say so on the page.

  3. Read the last-update date before you trust the phone number

53% of these records were last touched before 2020. A recent update is a good sign the phone and address are live; an old one means confirm online or with your referring office first.

  4. Call with the CPT code and say the words self-pay

Ask for the cash price for your exact code (70551 brain, 72148 lumbar spine, 73221 or 73721 joint MRI without contrast), whether the radiologist read is included, and whether the machine is 1.5T or 3T. Compare against the state range and any parsed hospital prices on the metro page.

  5. Check the two things the registry cannot tell you

Accreditation (ACR or IAC) and licensing are not in NPPES. Ask the centre directly, and use our quality checklist before booking on price alone.

Quality questions to ask: cheap MRI, good MRI checklist. Paying without insurance: cash-pay guide.

The Hospital vs Freestanding-Centre Gap

Verified figures from this site's Texas guides: hospital cash prices parsed directly from each hospital's federal price-transparency file (retrieved August 2026), against published imaging-programme rates in the same city (retrieved 3 August 2026).

CityHospital cash price (brain MRI)Published imaging-programme rateGap
Houston$1,335.50 Houston Methodist$263.225.1x
Dallas$2,324.00 Methodist Dallas$300.007.7x
Austin$2,556.77 Baylor Scott & White Austin$263.229.7x
San Antonio$2,743.79 Methodist Hospital$239.9511.4x

Imaging-programme rates: RadiologyAssist published city rate pages (cervical spine or brain MRI without contrast, radiologist report included). Hospital cash prices: each hospital's own machine-readable file published under the federal Hospital Price Transparency rule, parsed directly by us and retrieved August 2026. File dates: Houston Methodist 1 April 2026; Methodist Dallas 20 March 2026; Baylor Scott & White Austin 13 June 2026; Methodist Hospital San Antonio 1 May 2026. Imaging Centre Directory, Frequently Asked Questions

The August 2026 NPPES file holds 5,914 fixed-site organisations registered under the MRI clinic/center (261QM1200X) or radiology clinic/center (261QR0200X) taxonomy codes, plus 6,346 diagnostic-radiology organisations whose registered name or trading name identifies them as an imaging provider, 12,260 records in total after folding 473 duplicate NPIs. 1,747 carry the MRI-specific code. Mobile X-ray units, mammography-only sites and hospital departments registered under a radiology code are excluded from this directory.

Source: NPI registry records, status as reported by CMS, August 2026 file (NPPES data dissemination file dated 10 August 2026, download.cms.gov; US federal public data, no charge). NPPES is an identifier registry, not a licence register: an NPI does not validate licensure, accreditation or that a scanner is on site. Names, addresses and phones are as self-reported to CMS and many records have not been updated for years; confirm before you travel. Coordinates: US Census Bureau geocoder, ZIP-code centroid where the address did not match (flagged). Metro areas: 2023 Census core-based statistical areas by county. Price ranges: our methodology; hospital cash prices: each hospital's federal transparency file (45 CFR 180), where parsed. No centre pays to be listed and none is endorsed.
